Edward Teach, Raktul, and myself have an exp perma that we take part in every weeknight from 9pm est to midnight. During this grind we like yourself have had plenty of opportunities to test out battle regimens. The general consensus is that they simply don't work. Never the less we seemed to have found a round about way to guarantee a successful regimen. Before I get into it here is a small summary of regimens.
A regimen is a skillchain between two or more party members.
A regimen is initiated by Player 1 by clicking SHIFT + 1 (normal attack) or any other attack/spell. That ability is then queued up into a list that everyone in the party can see.
Other party members precede to queue up in a pre-disposed order (vent is an absolute must to execute these well), and their abilities are added to the list.
Once ready, in theory, all Player 1 needs to do is push G-key once. This will cause an animation spiral around each participating member and the battle regimen will execute.
Assuming no one misses their WS/attack. The chat log should read out "The monster is now under the effect of Ruin".
Sadly in practice it's not quite so straightforward. In fact after Player 1 pushes G, the second member of the regimen simply doesn't execute his ability. We played around with timing, and read a bunch of bunk theories on the subject but ultimately discovered our own clunky way of doing things.
The trick is to have a minimum of three members involved in the regimen. Lets use our party as an example.
1) Myself - MRD - Light Swing (normal attack)
2) Ed - GLD - Light Slash (normal attack)
3) Raktul - CNJ - irrelevant attack or spell
In the following chain. After I push G. Myself and Ed execute our abilities and form RUIN. Meanwhile Raktul never processes his attack and remains in Regimen mode. This is clearly a bug involving stack unwinding, but the third member is a required dummy in order to perform the regimen.
Now for the good news.
Performing RUIN is imperative in combat. It literally doubles the dmg output of all members attacking the target and creates a huge difference in combat. Our normal attacks at the time on Fat Dodos went from 47 to 99.
Keep in mind that during a regimen you are immobile and this can potentially get people killed.
We attempted combining WS -> WS using the same strategy but despite several misses and a number of combinations we failed to come up with any effects in the log.

During an exp session bige and I discovered we both have to push BR twice in order for regimen to go off. Bige br>bige ws add> Goku br> goku ws add> Bige hits br again> Goku hits br again. It went off every time like that but we were trying ws so we never got an affect.

I think that's the same conclusion that this guide (http://ffxivarchives...le-regimens-r31) came to as well.
- Click the Battle Regimen Button
- Select an Ability
- Wait for other member to queue their ability
- One person now clicks the Battle Regimen button to start the Regimen
- When you see the red glow you click for a second time activating the Regimen
